Fatal Crash on US-101 N Peninsula Ave Off-Ramp in San Mateo

San Mateo, CA – In the early hours of Sunday, October 26, 2025, a driver was killed in a single-vehicle crash on the US-101 North off-ramp at Peninsula Avenue, according to CHP Fatal.

Authorities report that the incident occurred around 4:15 AM when a four-door BMW vehicle went off the roadway after colliding with a roadside guardrail for unknown reasons. The driver was pinned inside the vehicle and found unconscious by emergency responders upon arrival.

Fire department personnel worked to extricate the individual from the vehicle, but the driver was pronounced dead at 4:29 AM. The vehicle ended up off the freeway onto city streets following the collision.

The off-ramp was closed for several hours while the California Department of Transportation assisted with a hard closure and investigators documented the scene. 

The coroner responded to the site to handle the fatality, and tow services removed the vehicle for further examination. 

Authorities continue to investigate the circumstances surrounding the crash.
