Fatal Multi-Vehicle Crash on US Hwy 101 near Freitas Pkwy in San Rafael

San Rafael, CA – On the afternoon of Monday, October 6, 2025, a multi-vehicle collision on U.S. Highway 101 left at least one person dead, according to CHP Fatal.

The California Highway Patrol reports that the crash took place around 4:33 PM on the southbound lanes of U.S. 101 near the Freitas Parkway on-ramp.

Investigators said three vehicles were involved in the collision, including a gray Ford Explorer, a gray Honda CR-V, and a gray Audi. The impact caused major front-end and rear-end damage, with heavy smoke reported from one vehicle. All airbags deployed in the middle vehicle, and multiple lanes were blocked as emergency crews arrived.

San Rafael Fire Department and Caltrans responded to the scene, closing all southbound lanes for an extended period under a SigAlert. Tow trucks were called for all three vehicles, and the freeway was reopened later in the evening after investigators and cleanup crews cleared debris.

Authorities confirmed a fatality at the scene. The identity of the deceased has not been released, and the cause of the crash remains under investigation by CHP.
